Проблемы лечения перитонитов, осложняющих хронический перитонеальный диализ

Аннотация: Несмотря на значительный прогресс в области терапевтической эффективности и техники перитонеального диализа (ППД), наиболее частым осложнением этого вида заместительной почечной терапии остаются перитониты. В то время как, по данным ряда авторов, их число заметно снизилось, другие исследователи по-прежнему отмечают высокую частоту этого осложнения. Фактически перитониты являются главной причиной коморбидности и прекращения перитонеального диализа [1]. Более того, недавно опубликованы сообщения, в которых подчеркивается связь между перитонитами и смертностью [2, 3]. И в новом тысячелетии проблема диализных перитонитов порождает ряд вопросов. Один из них касается их эмпирической терапии и обоснования ее стратегии. Второй - антибактериальной терапии у больных, получающих ночной циклический перитонеальный диализ, используемый все чаще в связи с необходимостью поддержания адекватных диализных клиренсов. Третий вопрос - каковы факторы риска перитонитов? Наконец, известно, что грамотрицательные перитониты прогностически тяжелее грамположительных. В связи с этим возникает вопрос: что мы знаем об этиологии грамотрицательных перитонитов? В какой степени сам катетер способствует развитию перитонитов? * Перевод А.М. Андрусева
