Земченкова Полноценное участие в жизни, которое требует физической активности и физических функций, является ключевым результатом лечения, о чем сообщают сами пациенты, получающие перитонеальный диализ (ПД). Для удовлетворения специфических потребностей этой группы пациентов требуется клиническое руководство из междисциплинарных источников относительно рекомендаций по физическим упражнениям и активности. С августа 2020 года по июнь 2021 года члены Глобальной почечной сети по физическим упражнениям (Global Renal Exercise Network) и Международного общества перитонеального диализа (International Society for Peritoneal Dialysis) проанализировали опубликованную литературу и международный клинический опыт для разработки набора клинических практических рекомендаций. Список вопросов, относящихся к физической активности и физическим упражнениям, был разработан с точки зрения пациента, получающего ПД, и эти вопросы послужили основой для разработки практических советов. Система GRADE использовалась для оценки качества доказательств и руководства по клинической практике. Обзор литературы выявил недостаточное качество доказательств, и, таким образом, рекомендации по клинической практике, как правило, основаны на экспертном мнении пациентов, получающих ПД, клиницистов-экспертов по упражнениям для пациентов на ПД и опытных исследователей физической активности при ПД. Практические советы касаются времени и условий начала выполнения упражнений и физической активности (после имплантации катетера, с диализатом или «сухим» животом), специфических видов деятельности (работа, секс, плавание, упражнения на мышцы центральной стабилизации - мышцы кора), потенциальных неблагоприятных исходов, связанных с физической активностью и упражнениями (уход за местом выхода катетера, потливость, заболевания сердечно-сосудистой системы, усталость, внутрибрюшное давление), влияния упражнений и физической активности на представляющие интерес аспекты состояния (психическое здоровье, ожирение, слабость, плохая физическая форма), а также питания при физических нагрузках.</p></abstract><trans-abstract xml:lang="en"><p>Life participation requiring physical activity and physical function is a key patient-reported outcome for people receiving peritoneal dialysis (PD). Clinician guidance is required from multidisciplinary sources regarding exercise and activity advice to address the specific needs of this group. From August 2020 through to June 2021, the Global Renal Exercise Network and the International Society for Peritoneal Dialysis reviewed the published literature and international clinical experience to develop a set of clinical practice points. A set of questions relevant to physical activity and exercise were developed from the perspective of a person receiving PD and were the basis for the practice point development. The GRADE framework was used to evaluate the quality of evidence and to guide clinical practice points. The review of the literature found sparse quality evidence, and thus the clinical practice points are generally based on the expert consensus of people receiving PD, PD exercise expert clinicians and experienced PD exercise researchers.
