Для комплексной оценки зуда и его влияния на разные аспекты качества жизни у больных с ХБП применяют специальные опросники, заполняемые пациентами. В настоящее время отсутствуют русскоязычные версии стандартизированных инструментов, прошедших апробацию на отечественной популяции пациентов с ХБП и пригодных для оценки кожных проявлений заболевания и их влияния на качество жизни пациентов данного профиля. Цель исследования: апробация применения опросников для комплексной оценки зуда и различных аспектов функционирования у больных ХБП, получающих ЗПТ, а также валидация русскоязычных версий опросников Itch MOS и Skindex-16. Методы: апробацию опросников Skindex-16, Itch MOS, 5D-шкалы Elman и WI-NRS осуществляли в группе взрослых пациентов с ХБП, находящихся на ЗПТ, во время их госпитализации. Дополнительно в рамках тестирования русскоязычные версий опросников Skindex-16, Itch MOS и 5D-шкалы Elman проводили интервьюирование больных в фокусных группах (n=5-8) для анализа понятности и легкости их заполнения пациентами. Оценку надежности выполняли на основании определения внутреннего постоянства (вычислением коэффициента Кронбаха-альфа) и воспроизводимости опросников методом тест-ретест. Для определения валидности оценивали внешнюю, содержательную и конвергентную валидность. Дополнительно для тестирования опросников проводили анкетирование специалистов. Статистический анализ проводили с использованием t-критерия Стъюдента, парного t-критерия или их непараметрических аналогов. Для оценки связи между значениями показателей опросников использовали корреляции Спирмена. Результаты: выполнена языковая и культурная адаптация и валидация опросников Skindex-16 и Itch MOS. Русскоязычные версии опросников характеризовались удовлетворительными показателями надежности и валидности. В апробацию опросников включены 100 пациентов с ХБП 5 стадии, получающих ЗПТ гемодиализом на протяжении 49,5 (Q1-Q3: 16; 79) месяцев. В тестировании опросников приняли участие 6 специалистов. На основании данных апробации показано, что все использованные опросники удобны, информативны, клинически интерпретируемы и целесообразны для совместного использования. Выводы: разработаны русскоязычные версии опросников Skindex-16 и Itch MOS; продемонстрированы их надежность и валидность для оценки проблем, связанных с кожным зудом, у больных ХБП, находящихся на гемодиализе. Апробированные в отечественной популяции больных ХБП, получающих ЗПТ, русскоязычные версии опросников Skindex-16, Itch MOS, 5D-шкалы Elman и WI-NRS могут применяться в научных исследованиях и клинической практике для анализа разных аспектов кожного зуда и связанных с ним нарушений качества жизни в этой популяции пациентов.</p></abstract><trans-abstract xml:lang="en"><p>Rationale: itching is one of the essential problems in patients with chronic kidney disease (CKD), including those receiving renal replacement therapy (RRT). For a comprehensive assessment of itching and its impact on various aspects of the quality of life in patients with CKD, questionnaires filled out by patients are used. Currently, there are no Russian versions of standardized instruments that have been tested on the domestic population of patients with CKD and are suitable for assessing the skin manifestations and their impact on the quality of life of this patient population. The aim of the study was to test the use of questionnaires for a comprehensive assessment of itching and various aspects of functioning in CKD patients receiving RRT, as well as validation of the Russian versions of the Itch MOS and Skindex-16 questionnaires. Methods: the Skindex-16, Itch MOS, 5D Elman scale, and WI-NRS scale questionnaires were tested in a group of adult patients with CKD who were on RRT during their hospitalization. To test Russian versions of the Skindex-16, Itch MOS, and 5D-Elman questionnaires, patients were interviewed in focus groups (n=5-8) to analyze the clarity and ease of filling them out by patients. Reliability was assessed by internal consistency (Cronbach's alpha coefficient) and by the test-retest method. Content, face, and convergent validity were evaluated. Also, specialists were interviewed to test the questionnaires. Statistical analysis was performed using the Student's t-test, paired t-test, or their nonparametric analogues. Spearman correlations were used to assess the relationship between the values of the questionnaire indicators. Results: the linguistic and cultural adaptation and validation of the Skindex-16 and Itch MOS questionnaires were performed. The Russian versions of the questionnaires were characterized by satisfactory reliability and validity. To test the use of the questionnaires among patients 100 patients with stage 5 CKD receiving HRT hemodialysis for 49.5 (Q1-Q3: 16; 79) months were included. 6 specialists took part in the testing of the questionnaires. All the questionnaires were shown to be convenient, informative, clinically interpretable, and appropriate for joint use. Conclusion: Russian versions of the Skindex-16 and Itch MOS questionnaires have been developed; their reliability and validity for assessing concerns associated with itching in patients with CKD undergoing hemodialysis have been demonstrated.
