В контрольную группу были включены 723 пациента с трансплантатами от доноров, умерших от черепно-мозговой травмы - I группа. Группу исследования составили 41 реципиент трансплантата от маргинальных доноров, умерших от шока - II группа. Средний срок наблюдения составил 52 ± 19 месяцев. В обеих группах сравнивали следующие параметры: частоту отсроченной функции и острой реакции отторжения трансплантата, степень ишемически-реперфузионных повреждений (по данным биопсии), частоту первично нефункционирующих трансплантатов, почечную функцию за период наблюдения, а также 5-летнюю выживаемость реципиентов и трансплантатов. Во II группе выявлен больший процент отсроченной функции (67% против 51%) и первично нефункционирующих трансплантатов (7 против 4%), р &lt; 0,05. 5-летняя выживаемость трансплантатов и реципиентов во II группе оказалась ниже, чем в I (67 против 73% и 71 против 78% соответственно), р &lt; 0,05. Уровень сывороточного креатинина к моменту окончания наблюдения во II группе оказался выше, чем в I (165 ± 80 и 151 ± 50 соответственно), р &gt; 0,05. Мы пришли заключению о том, что, несмотря на несколько худшие результаты трансплантаций почек от крайне нестабильных доноров, умерших от шока, стратегия их использования, особенно учитывая длительность периода ожидания трансплантации, имеет перспективы.</p></abstract><trans-abstract xml:lang="en"><p>We evaluated our results of kidney transplantation from marginal donors who had dopamine-dependent unstable haemodynamics and died from traumatic or haemorrhagic shock. The control group I of 723 patients (with kidney transplanted from donors who died from craniocerebral injury) was compared to group II of 41 patient with kidneys transplanted from the marginal donors. The followed up period was 52 ± 19 months. In both groups the percent of dilate graft function, primary non function transplants, acute graft rejection, patients and graft survival, biopsy prove ischemic-reperfusion graft injury were evaluated. In group II the percent of dilate graft function, primary non function transplants was higher (67% and 51%, 7 and 4%, respectively). 5-years grafts and patients survival rates in group II were lower than in group I (67 and 73%, 71 and 78%, respectively, р &lt; 0,05). At the end of the follow up period the level of serum creatinine was 151 ± 50 mM in group I and 165 ± 80 mM in group II.
