Обследовано 102 больных (40 мужчин и 62 женщины) с хронической болезнью почек 1-3 ст. (по классификации NKF K/DOQI, 2002 г). Средний возраст больных составлял 44,2 ± 8,3 года, средняя СКФ - 75,0 ± 1,9 мл/мин. СКФ рассчитывали по формуле Кокрофта-Голта. Гипертрофию левого желудочка (ГЛЖ) диагностировали при индексе массы миокарда (ИММЛЖ) ³134 г/м2 у мужчин и ИММЛЖ ³110 г/м2 у женщин. Почечный сосудистый кровоток исследовали при помощи допплерометрии почечных сосудов со спектральным анализом на уровне сегментарных и междолевых артерий. У больных с бόльшим уровнем почечного сосудистого сопротивления регистрировалась более тяжелая артериальная гипертензия, почти в 5 раз чаще наблюдалась ГЛЖ и ХСН различной степени тяжести и почти в 4 раза чаще - ИБС. Наряду с традиционными факторами риска, выявлена прямая связь между ИММЛЖ и индексом резистентности (RI) и пульсационным индексом (PI) междолевых артерий (r = 0,7, p &lt; 0,01 и r = 0,57, p &lt; 0,01 соответственно) и обратная связь между ИММЛЖ и конечным диастолическим кровотоком (Vmin), ИММЛЖ и СКФ (r = -0,53, p &lt; 0,05 и r = -0,34, p &lt; 0,05 соответственно).</p></abstract><trans-abstract xml:lang="en"><p>There are structural and functional changes in myocardium of patients with chronic kidney disease (CKD) which cause high cardiovascular morbidity and mortality. The aim of this study was to reveal the correlations between the left ventricular hypertrophy (LVH), duration of hypertension, GFR and renal vascular resistance in patients on early stages of CKD. 102 patients (40 males, 62 females, mean age 44,2 ± 8,3 years) with CKD 1-3 stages (by NKF K/DOQI classification, 2002) were included in the study. The GFR was estimated by the serum creatinine level using the Cockcroft-Gault formula; left ventricular hypertrophy was determined according to echocardiography criteria. We also examined renal vascular resistance by Doppler ultrasonography in segmentar and intralobal arteries of kidneys. We diagnosed LVH and chronic heart failure in patients with higher renal vascular resistance 5 times more frequently and ischemic heart disease 4 times more frequently than in patients with normal renal resistance.
