Целью исследования явилось уточнение связи между нарушением функции почек, анемией и функциональным состоянием сердца у больных пожилого возраста с хронической сердечной недостаточностью (ХСН). Методы. Обследовано 112 больных с ХСН I-IV функционального класса в возрасте 65-88 лет (средний возраст 76,8 ± 5,1 лет). Женщин было 71, мужчин - 41. Анемию диагностировали при уровне гемоглобина у мужчин &lt;130 г/л, у женщин &lt;120 г/л. Клубочковую фильтрацию (КФ) рассчитывали по формуле Cockcroft-Gault. Всем больным выполнена эхокардиография и допплер-эхокардиография. Результаты. Клубочковая фильтрация ниже 60 мл/мин/1,73 м2 наблюдалась у 43 больных (38,4%). Анемия диагностирована у 31 пациента (27,7%). Сочетание КФ ниже 60 мл/мин/1,73 м2 и анемии наблюдалась у 21 больного с ХСН (18,8%). У больных с I-II ФК ХСН клубочковая фильтрация была выше, чем у пациентов с III-IV ФК ХСН (82,5 ± 28,5 и 62,4 ± 19,1 мл/мин/1,73 м2 соответственно, p &lt; 0,001). Гемоглобин также был выше у больных с ХСН I-II ФК, чем у больных с III-IV ФК ХСН (132,4 ± 12,9 и 125,2 ± 12,7 г/л соответственно, p &lt; 0,01). Наблюдалась прямая связь между КФ и содержанием гемоглобина (r = 0,3; p &lt; 0,01). ФВ &lt; 45% была у 17 (15,2%) больных. Гипертрофия левого желудочка диагностирована у 103 (92,0%) больных. Установлено наличие независимой связи между степенью нарушения функции почек (уровень КФ) и нарушением релаксации - чем ниже КФ, тем больше время изоволюмического расслабления левого желудочка. Обнаружена обратная независимая связь между концентрацией гемоглобина и скоростью раннего диастолического наполнения. Заключение. У 112 пожилых больных с ХСН клубочковая фильтрация ниже 60 мл/мин/1,73 м2 наблюдается в 38,4% (43), анемия по критериям ВОЗ - в 27,7% (31 пациент), кардиоренальный анемический синдром - у 21 (18,8%) пациента. Помимо структурных изменений сердца на диастолическое наполнение влияют снижение функции почек и анемия, а также уровни систолического и диастолического артериального давления. Неблагоприятное влияние сниженной функции почек на течение ХСН обусловлено ухудшением диастолической функции.</p></abstract><trans-abstract xml:lang="en"><p>Aim. The aim of this study was to evaluate the relationship between renal dysfunction, anemia and cardiac function in elderly patients with chronic heart failure (CHF). Methods. 112 patients 65 years old or older (F 71, M 41, mean age - 76,8 ± 5,1 years) with chronic heart failure I-IV class (NYHA) were studied. Anemia was defined by the World Health Organization criteria (hemoglobin level &lt;130 g/l for men and &lt;120 g/l for women). Glomerular filtration rate (GFR) was calculated by the Cockcroft-Gault formula. Results. GFR was below 60 ml/min/1,73 m2 in 43 (38,4%) patients; 31 (27,7%) patients had anemia. GFR below 60 ml/min/1,73 m2 and cardio-renal-anemia syndrome was found in 21 (18,8%) patients with CHF. GFR was higher in patients with I-II NYHA class than in patients with III-IV class (82,5 ± 28,5 и 62,4 ± 19,1 ml/min/1,73 m2; respectively, p &lt; 0,001). Hemoglobin was higher in patients with I-II class of CHF than in patients with III-IV class (132,4 ± 12,9 и 125,2 ± 12,7 g/l; respectively, p &lt; 0,01). A positive correlation was found between GFR and hemoglobin concentration (r = 0,3; p &lt; 0,01). Ejection fraction of left ventricular was &lt;45% in 17 (15,2%) patients. Left ventricular hypertrophy was detected in 103 (92,0%) patients. Multiple regression analysis showed independent positive relationship between GFR and relaxation time of left ventricular. A negative correlation between hemoglobin concentration and peak-E velocity was found. Conclusions.
