Интрадиализная гипотензия (ИДГ) остается частым осложнением у больных на программном гемодиализе (ГД). Целью исследования явилось уточнение связи между ИДГ и диастолической дисфункцией левого желудочка. Методы. Обследовано 20 больных (11 мужчин, 9 женщин, средний возраст - 49 ± 12 лет), находящихся на бикарбонатном гемодиализе (4 часа × 3 раза в неделю). Причинами ХПН были хронический гломерулонефрит (n = 9), гипертоническая болезнь (n = 9), сахарный диабет (n = 2). Двенадцать больных имели хроническую сердечную недостаточность, 17 - артериальную гипертензию. По результатам эхокардиографии рассчитывали фракцию выброса (ФВ) и индекс массы миокарда левого желудочка (ИММЛЖ). Трансмитральный кровоток оценен методом допплерэхокардиографии. Определяли следующие параметры наполнения левого желудочка: максимальную скорость раннего диастолического наполнения (Е), максимальную скорость наполнения в систолу предсердий (А), отношение этих скоростей (Е/А), время изоволюмического расслабления (IVRT), время замедления потока раннего диастолического наполнения (DT). Все измерения выполнены одним исследователем до и после ГД. Результаты. Гипертрофия левого желудочка (ГЛЖ) диагностирована у 18 больных. Фракция выброса была ниже 45% у 2 больных. Ультрафильтрация составила 1,9 ± 1,2 (0,2-4,2) литра. Обнаружена достоверная положительная корреляция между снижением веса за сеанс ГД и ∆E (r = 0,58; p = 0,01). При этом не отмечено связи между ∆E и ИДГ. Показатель DT был значительно выше у больных с ИДГ (231,2 ± 65,3 мс), чем у больных со стабильной гемодинамикой (175,0 ± 51,0 мс, p = 0,04). Заключение. Развитие интрадиализной гипотензии ассоциировано с наличием у больного ГЛЖ со значительным увеличением времени замедления раннего диастолического потока левого желудочка.</p></abstract><trans-abstract xml:lang="en"><p>Aim. Intradialytic hypotension remains a frequent complication of dialysis. The aim of this study was to estimate the relationship between diastolic dysfunction and intradialytic hypotension in chronic hemodialysis patients. Methods. Twenty dialysis patients (11 M, 9 F, mean age 49 ± 12 years) who receive 4 hours bicarbonate HD 3 times a week were studied. Causes of chronic renal failure were chronic glomerulonephritis (n = 9), essential hypertension (n = 9) and diabetes mellitus (n = 2). Twelve patients had chronic heart failure. Seventeen patients had arterial hypertension. M-mode echocardiography was performed and left ventricular mass index (LVMI), ejection fraction (EF) were calculated. Transmitral flow was assessed by Doppler echocardiography. Peak velocity of early (E) and late (A) filling, E/A ratio, isovolumic relaxation time (IVRT) and early deceleration time (DT) were detected. All measures were performed one hour before and immediately after HD by one investigator. Results. Left ventricular hypertrophy was detected in 18 patients. EF was lower than 45% in 2 patients. Average amount of ultrafiltration was 1,9 ± 1,2 (range 0,2-4,2) l. There was significant positive correlation between weight loss and ∆E (r = 0,58; p = 0,01), and we didn’t detect correlation between ∆E and intradialytic hypotension. Patients with intradialytic hypotension had higher DT than patients without IDH (231,2 ± 65,3 vs 175,0 ± 51,0 ms, respectively, p = 0,04). Conclusion.
