Введение Нодулярный мезангиальный склероз является очень характерной, но не специфической находкой для диабетической нефропатии. Кроме диабетической нефропатии, нодулярный мезангиальный склероз может наблюдаться при болезни отложения моноклональных иммуноглобулинов (MIDD - monoclonal immunoglobulin deposition disease), обычно при продукции моноклональных легких цепей, также при выраженном мембранозно-пролиферативном гломерулонефрите (МбПГН) и, реже, при врожденных патологиях сердца. Однако сочетание нодулярного мезангиального склероза с гломерулярным гиалинозом в виде фибриновых «чашек» (fibrin cap), с наличием каплевидного гиалиноза капсулы Боумена (capsular drops), c диффузным утолщением гломерулярной базальной мембраны и выраженным артериологиалинозом считается специфичным для диабетической нефропатии. В данной статье мы представляем случай, который является уникальным из-за того, что нодулярный мезангиальный склероз в сочетании с другими характерными для диабетической нефропатии находками возник в трансплантате почки реципиента без клинических признаков сахарного диабета через 21 год после трансплантации. * Печатается с разрешения Oxford University Press, перевод А.В.
