Введение Перитонеальными симптомами раздражения брюшины у пациентов, получающих постоянный амбулаторный перитонеальный диализ (ПАПД), обычно проявляются диализные перитониты при отсутствии или наличии перфорации кишки. Тем не менее ишемическая болезнь кишечника (ИБК) должна быть принята во внимание при проведении дифференциального диагноза. Эта острая мезентериальная катастрофа, которая может быть расценена как диализный перитонит из-за схожих проявлений [1]. ИБК может быть разделена на окклюзивную и неокклюзивную [2]. Неокклюзивный мезентериальный инфаркт был впервые описан Ende в 1958 г. у пациента без поражения почек с сердечной недостаточностью [3]. В последние годы у пациентов, получающих гемодиализ, эпизоды гипотензии довольно часто ассоциируются с выявлением мезентериальных инфарктов [4]. Возможно, что в связи с меньшим риском эпизодов гипотензии у пациентов, получающих ПАПД, реже сообщается о случаях мезентериальных инфарктов у них. Мы наблюдали двух пациентов с терминальной ХПН, получавших лечение ПАПД, у которых развилась ИБК. ** Печатается с разрешения Oxford University Press; перевод с англ. А.М. Андрусева.
