В период с 1996 по 2001 гг. наблюдались 669 больных с терминальной стадией хронической почечной недостаточности (тХПН), получавших лечение в четырех центрах гемодиализа Москвы. Маркеры перенесенной или текущей инфекции, вызванной вирусом гепатита В (HBV), были выявлены у 411 пациентов (61%). У 16 (2%) имелись признаки острого гепатита В (ОГВ). У 80 больных (12%) определялось носительство HВsAg, из них в сочетании с HCV-инфекцией - у 21 пациента (3%). Среди больных, в крови которых были обнаружены только специфические антитела к антигенам HBV, преобладали случаи с изолированными HBcoreAb (у 55%). HBsAb были выявлены у 21% пациентов, а их сочетание с HBcoreAb - у 24%. Подтверждена связь заражения HBV с продолжительностью лечения программным гемодиализом (ПГД). Основная часть больных (76%) оказалась инфицирована в течение первого года после начала заместительной почечной терапии. Не удалось установить связь заражения с полом и возрастом пациентов, характером заболевания почек, ставшего причиной развития уремии, а также с количеством произведенных гемотрансфузий. Течение ОГВ в условиях лечения ПГД отличалось преимущественно субклиническими проявлениями. Часть больных, несмотря на проводимую вакцинацию, оказалась инфицирована HBV. В этой группе ОГВ характеризовался большей частотой желтушных форм заболевания. Хронизация гепатита В установлена у 37% пациентов. У 77% больных с персистированием HBsAg болезнь протекала с признаками репликации вируса.

Between 1996 and 2001 669 patients with end stage of renal disease, treated in four Moscow Hemodialysis Units were observed. Markers of in past or current infection, caused by hepatitis В virus (HBV) were revealed in 411 patients (61%). In 16 patients (2%) the signs of acute hepatitis В (AHB) were found. 80 patients (12%) proved to be carriers of HBsAg, and in 21 of them (3%) there was a combination with HCV-infection. Among the patients with detected specific antibodies to HBV antigens, the isolated HBcoreAb were found most often (in 55%). HBsAb were revealed in 21% of patients, and their combination with HBcoreAb - in 24%. The linkage between the duration of hemodialysis treatment and HBV-infection was confirmed. Most of patients (76%) proved to be infected during the first year of replacement renal therapy. But we did not succeed in establishing a connection between frequency of infection and sex, age of patients, the character of renal disease which caused uremia, and the quantity of hemotransfusions. The course of AHB in hemodialysis patients was characterized by preferentially subclinical manifestations. Some patients, in spite of vaccination, proved to be infected by HBV. In this group AHB was characterized by higher frequency of jaundice. Hepatitis В chronification was established in 37% of patients. In 77% of patients with persistence of HBsAg, the signs of viral replication were found.
