Но несмотря на этот прогресс, ГПТ остается частым следствием хронической почечной недостаточности и требует хирургического удаления паращитовидных желез, особенно в тех случаях, когда ГПТ становится автономным и может быть расценен как «третичный гиперпаратиреоз». Сложные взаимодействия между эндокринными, экзокринными и пищевыми факторами мешают выработать эффективную стратегию снижения повышенного уровня паратиреоидного гормона (ПТГ) и лечения сопутствующих осложнений. Последствия неадекватного контроля уровня ПТГ часто бывают тяжелыми, а в некоторых случаях, таких как кальцифилаксия, и опасными для жизни. Последние исследования позволяют предположить, что уровень ПТГ порядка 800 пг/мл ассоциирован со снижением выживаемости [<xref ref-type="bibr" rid="cit1">1</xref>]. Эти данные согласуются с другими результатами, которые указывают, что висцеротоксичность ПТГ в сочетании с его повреждающим воздействием на кости, вносит прямой вклад в развитие многих патологических состояний у пациентов с хронической почечной недостаточностью.
