Представлено клиническое наблюдение кардиопульмонального поражения при амилоидозе у пациента с ранее установленным диагнозом семейной средиземноморской лихорадки (ССЛ), осложнившейся амилоидозом почек и диализпотребной терминальной почечной недостаточностью. Ключевыми позициями диагностики были: эхокардиографические признаки рестриктивной кардиомиопатии, рентгенологические признаки диссеминированного паренхиматозного процесса в легких и высокая легочная гипертензия. В клинической картине обращало на себя внимание отсутствие признаков дыхательной и сердечной недостаточности. Дальнейший диагностический поиск позволил исключить наличие у пациента туберкулеза, саркоидоза, системного заболевания соединительной ткани и онкологической патологии в качестве вероятных причин сочетанного поражения сердца и легких. Методом исключения данный процесс был расценен как паренхиматозный нодулярный амилоидоз легких. Спустя 3 месяца, на фоне лечения колхицином, констатирована выраженная положительная динамика: полностью регрессировали признаки диссеминированного процесса в легких, нормализовались показатели систолического давления в легочной артерии, улучшилась диастолическая функция левого желудочка. Данное наблюдение демонстрирует клиническую необходимость поиска внепочечных локализаций амилоидоза у пациентов с ССЛ и подчеркивает важность патогенетической терапии даже на поздних стадиях заболевания.

The article presents a clinical case of amyloidosis cardiopulmonary lesions in a patient with previously established diagnosis of Familial Mediterranean Fever (FMF). In this patient FMF was complicated by renal amyloidosis and end-stage renal disease. Key points of diagnostics were high pulmonary hypertension, echocardiographic feature of restrictive cardiomyopathy and radiological signs of disseminated parenchymal process. The lung damage was interpreted as parenchymal nodular pulmonary amyloidosis after tuberculosis, sarcoidosis, systemic autoimmune diseases and oncology were excluded. The treatment by colchicine was started immediately. Significant positive changes were observed in three months. The signs of disseminated parenchymal process and pulmonary hypertension were completely resolved. The diastolic function of left ventricle also significantly improved. The described case demonstrates the clinical need to examine extrarenal localizations of amyloidosis in patients with FMF and emphasizes the importance of treatment by colchicine even at a late stages of disease.
