В 5 случаях были имплантированы внутрисосудистые протезы (стенты). У всех больных артериальное давление (АД) контролировалось по Короткову, определялся клиренс креатинина, выполнялось УЗИ сердца, почек, почечных артерий (ПА), а также ангиографическое исследование па до, сразу после и спустя 3, 6, 12 месяцев после БАП. Выявлена сопоставимость градиентов АД в месте стенозов ПА, измеряемых методом непрерывно-волновой допплерографии (НВДГ) и непосредственно во время ангиопластики. Показана возможность и целесообразность использования непрерывно-волновой допплерографии (НВДГ) для неинвазивной оценки эффективности ангиопластики почечных артерий, а также для скринингового выявления стенозов ПА. В процессе динамического наблюдения больных было установлено, что в результате успешной ангиопластики ПА, достигается гипотензивный эффект, сохраняющийся в течение не менее 1 года наблюдения. При этом по данным ЭхоКГ, отмечена тенденция к уменьшению толщины межжелудочковой перегородки (ТМЖП) и задней стенки левого желудочка (ЗСЛЖ). У больных с критическими стенозами ПА и исходно нарушенной функцией почек через 12 мес. после БАП отмечено достоверное повышение клиренса эндогенного креатинина и увеличение объемных размеров почки на стороне стеноза ПА.</p></abstract><trans-abstract xml:lang="en"><p>We observed 32 patients with vasorenal hypertension; the balloon angioplasty of renal arteries was performed in all patients; in 5 cases with stenting of artery. The 24-hours monitoring of blood pressure (by Korotkov method), functional renal indices, ultrasonic investigation of heart, kidneys and renal arteries and angiography of renal arteries were carried out before and after balloon angioplasty, in 3, 6 and 12 months. Our study shows the opportunity and expediency of continuous wave Doppler (CWD) as a qualitative non-invasive method of diagnostical screening and may be of great value in assessment of results of renal arteries angioplasty. We observed a strong correlation between blood pressure gradients, getting from CWD, and results of direct intra-arterial blood pressure measuring during the angioplasty. The analysis of ultrasonographical findings revealed satisfactory hipotensive effect of renal artery angioplasty, which has sustained during one year of observation. The tendency to decrease of thickness of the interventricular septum and the inferior wall of left ventricle was marked. In patients with critical stenosis of renal arteries, complicated with renal functions insufficiency, after successful balloon angioplasty we observed significant increase of glomerular filtration, evaluated by clearance of endogenic creatinin.
