Макроглобулинемия Вальденстрема (МВ) представляет собой В-клеточное лимфопролиферативное заболевание, которое, как и множественная миелома (ММ) относится к числу так называемых парапротеинемических гемобластозов. Нефротический синдром (НС) при МВ наблюдается редко. Его морфологическим субстратом могут быть амилоидоз, а также (реже) другие гломерулопатии. В литературе имеются лишь единичные сообщения о гломерулонефрите, осложняющем МВ. В настоящем сообщении представлен случай МВ, осложнившейся НС, обусловленным мезангиокапиллярным гломерулонефритом. Весьма вероятно, что в механизме последнего играет роль образование антител к IG M, продуцируемому клетками опухоли, и последующее отложение иммунных комплексов на базальной мембране клубочков.

We report here the case of Waldenstrom’s Macroglobulinaemia ( MW) complicated with mesangioproliferative glomerulonephritis (MPGN) which appeared in a woman of 76 years old. The main clinical features of the disease were nephrotic syndrom associated with vasculitis. The blood immunochemistry revealed a monoclonal secretion of IgM. The changes typical for MPGN were observed in kidney biopsy. We suppose that MPGN was caused by the deposition on the GBM of immune complexes containing the antibody against monoclonal IgM.
