Причинами смерти этих больных стали другие факторы: сердечно-сосудистые болезни, сепсис, онкологические заболевания и т.д. Факторы риска, связанные с внезапной сердечной смертью у больных на ПГД, лишь частично совпадают с таковыми в общей популяции – почечная недостаточность и гемодиализ подвергают этих пациентов повышенному риску внезапной сердечной смерти по сравнению с лицами с сохраненной функцией почек.</p><p>Целью нашей работы явилось выявление возможного фенотипа-предиктора пациентов, получавших заместительную почечную терапию на гемодиализе и погибших внезапно. Нами проведено исследование влияния эхокардиографических (ЭхоКГ), электрокардиографических (ЭКГ), лабораторных показателей на общую и кардиальную пятилетнюю выживаемость больных (n=212), находившихся на ПГД с 2001 г.</p></sec><sec><title>Результаты</title><p>Результаты. Данные 14 умерших больных (31,1% от всех кардиологических смертей), их обстоятельства гибели соответствовали определению внедиализной внезапной сердечной смерти. Нам удалось показать особенности пациентов, умерших внезапно на ПГД. Эхо-кардиографическим параметром, рассчитанным на старте исследования, значительно отличавшимся в группе пациентов с ВСС на ПГД, был конечный диастолический объем. Медиана этого показателя у больных с ВСС, независимо от ФВЛЖ была значительно больше, чем у пациентов других групп: 182,8 (144,9-226,2) мл в группе ВСС (n=14), 101,0 (75,0-129,3) мл – в группе ССП (n=52), 104,1 (101,2-167,0) – в группе других причин (n=45) и 114,2 (81,7-140,5) мл у пациентов, живущих в настоящее время (n=99) (р=0,007).</p><p>В сравнении с больными, летальность которых связана с другими причинами, это были более молодые пациенты, с дилатацией ЛЖ и небольшим диализным стажем.</p><p>Особенно короткой продолжительность жизни была у больных с ВСС и низкой ФВЛЖ &lt;40%. Таких больных на ПГД – часто моложе 55 лет, с увеличенным конечным диастолическим объемом, особенно при наличии низкой ФВЛЖ, с относительно небольшим стажем диализа следует считать группой риска ВСС на ПГД.</p><p>Результаты проведенной работы позволяют говорить о более высоком риске ВСС на ПГД у больных моложе 55 лет, с относительно небольшим стажем диализа и дилатацией левого желудочка сердца, особенно при наличии низкой ФВЛЖ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Kidney replacement therapy – primarily maintenance hemodialysis (MHD) – has enabled patients with end-stage kidney disease to survive for decades despite the loss of kidney function. Consequently, mortality in this population is now largely driven by other causes, including cardiovascular disease, infections, including sepsis, and cancer. Risk factors for with sudden cardiac death (SCD) in patients receiving MHD only partially overlap with those in the general population: renal failure and hemodialysis significantly increase the risk of SCD compared with individuals who have preserved renal function.</p><p>The aim of our study was to identify a possible predictor phenotype for SCD in patients undergoing MHD. We analyzed the impact of echocardiographic (EchoCG), electrocardiographic (ECG), and laboratory parameters on overall and cardiac five-year survival in prospective single center patients cohort (n=212) undergoing MHD from 2001 till 2025.</p></sec><sec><title>Results</title><p>Results: data from 14 deceased patients (31.1% of all cardiac deaths) met the criteria for non-dialytic SCD. We identified clinical profile characteristics of patients who experienced SCD during long-term MHD. The only echocardiographic parameter at baseline differed significantly in the SCD group was the end-diastolic volume (EDV). The median EDV in patients with SCD, regardless of left ventricle ejection fraction (LVEF), was markedly higher than in patients in other groups: 182.8 (144.9-226.2) ml in the SCD group (n=14), 101.0 (75.0-129.3) ml in the reduced LVEF group (n=52), 104.1 (101.2-167.0) ml in the other causes group (n=45), and 114.2 (81.7-140.5) ml in currently living patients (n=99, р=0.007). Compared with patients who died from other causes, those with SCD were younger, had LV dilation, and a relatively short dialysis vintage. Survival was particularly poor in SCD patients with LVEF &lt;40%. Patients undergoing MHD who are under 55 years of age, had increased EDV (especially when accompanied by reduced LVEF), and have a short dialysis history should be considered at elevated risk for SCD on MHD.</p></sec><sec><title>Conclusion</title><p>Conclusion: the results of this study indicate that patients younger than 55 years, with a with a relatively short dialysis history and left ventricular dilation, especially when LVEF is reduced, carry a higher risk of SCD on MHD.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>внезапная сердечная смерть</kwd><kwd>программный гемодиализ</kwd><kwd>хроническая болезнь почек</kwd></kwd-group><kwd-group xml:lang="en"><kwd>sudden cardiac death</kwd><kwd>scheduled hemodialysis</kwd><kwd>chronic kidney disease</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Бикбов Б., Томилина НА.
