Беременные с хроническим гломерулонефритом (ХГН) подвержены высокому риску развития плацента-ассоциированных и перинатальных осложнений. Представляем клиническое наблюдение благоприятного исхода беременности у пациентки с впервые диагностированным в первом триместре ХГН и протеинурией нефротического уровня, на фоне длительной поддерживающей комбинированной терапии гепарином и ацетилсалициловой кислотой.</p></sec><sec><title>Клиническое наблюдение</title><p>Клиническое наблюдение. Пациентка 24 лет в течение полутора лет до наступления беременности дважды перенесла эпизоды макрогематурии. Не обследовалась. В первом триместре беременности выявлена тенденция к формированию нефротического синдрома (НС), исключено вторичное поражение почек как причина развития протеинурии нефротического уровня. Диагностирован первичный ХГН, предложено прерывание беременности, от которого пациентка отказалась. Назначены дипиридамол до 13 недели и низкомолекулярный гепарин, с 13 недели дипиридамол заменен на ацетилсалициловую кислоту, выполнялся мониторинг теста тромбодинамики. На фоне проводимой терапии состояние пациентки оставалось стабильно удовлетворительным: отеки отсутствовали, АД до 120/80 мм рт.ст. без антигипертензивных препаратов, суточная протеинурия снизилась и стабилизировалась на уровне 1,5-1,6-1,7 г/л, эритроцитурия 10-20 в поле зрения, в крови креатинин 67-72 мкмоль/л, общий белок 55-59 г/л, альбумин 30-32 г/л. Маркеры преэклампсии в пределах референсного диапазона. Оперативное родоразрешение в 39 недель, родилась доношенная девочка 8/9 баллов по шкале Апгар. В послеродовом периоде развился НС, выполнена биопсия почки: фокальный глобальный и сегментарный гломерулосклероз. Лечение циклоспорином в течение полутора лет до достижения полной ремиссии НС. На текущий момент стойкая ремиссия НС, проводится только нефропротективная терапия лозартаном 50 мг/сут.</p></sec><sec><title>Заключение</title><p>Заключение. Динамический мультидисциплинарный мониторинг, терапия гепарином и ацетилсалициловой кислотой повышает вероятность благоприятного исхода беременности у пациенток с ХГН.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Pregnant women with chronic glomerulonephritis (CGN) are at high risk of placenta-associated and perinatal complications. We present a clinical case of successful pregancy outcome in a patient with CGN and nephrotic syndrome (NS), diagnosed in the first trimester, achieved through long-term maintenance combination therapy with heparin and acetylsalicylic acid.</p></sec><sec><title>Clinical observation</title><p>Clinical observation. A 24-year-old woman experienced two episodes of macrohematuria within one and a half years prior pregnancy, but was not evaluated at that time. A tendency towards the development of nephrotic syndrome (NS) was identified in the first trimester of pregnancy; secondary causes of kidney disease was excluded. Primary CGN was diagnosed. Although termination of pregnancy was recommended, the patient declined. Dipyridamole and low-molecular-weight heparin were prescribed until 13 weeks gestation; from 13 week onward, dipyridamole was replaced with aspirin, and thrombodynamic parameters were monitored. The patient's condition remained stable and satisfactory throughout pregnancy: no edema, blood pressure up to 120/80 mm Hg, without antihypertensive therapy, daily proteinuria 1.5-1.7 g/L, erythrocyturia 10-20 per high-power field, blood creatinine 67-72 μmol/L, total protein 55-59 g/L, albumin 30-32 g/L. Markers of preeclampsia within the reference range. At 39 weeks of gestation, operative delivery was performed. A full-term female infant was born with Apgar scores of 8/9. In the postpartum period, NS recurred, and a kidney biopsy was performed revealing focal global and segmental glomerulosclerosis. Cyclosporine therapy was administered for one and a half years, resulting in complete remission of NS. Currently, there patient remains in stable remission of the NS and receives nephroprotective therapy with losartan 50 mg/day.</p></sec><sec><title>Conclusion</title><p>Conclusion.
