Беременность у пациенток с терминальной почечной недостаточностью (ТПН), находящихся на постоянном лечении программным гемодиализом (ПГД), до сих пор остается сложной и редкой клинической ситуацией. Несмотря на тенденцию к увеличению процента благополучных исходов, течение подобной беременности сопряжено с высоким риском гестационных осложнений как у матери, так и у плода. Представленная статья посвящена описанию подобного уникального клинического случая - успешного завершения беременности у пациентки с ТПН, более 10 лет находящейся на лечении ПГД. В обзоре литературных данных обсуждаются вопросы, касающиеся планирования, тактики ведения беременности, включающей изменения режима ПГД, коррекции анемии, нарушений фосфорно-кальциевого обмена и артериальной гипертензии, назначения антикоагулянтной терапии и профилактики возможных осложнений у беременных с ТПН, получающих лечение ЗПТ (ПГД).

Pregnancy in a woman with end-stage renal disease (ESRD) on long-term hemodialysis is a difficult and rare clinical situation. Such pregnancy is associated with increased maternal and fetal complications. We report a case of successful management of a pregnancy in a woman with end-stage renal disease and receiving hemodialysis for more than 10 years. We also reviewed planning, pregnancy management, dialysis strategy, problems of anemia, fluid balance, secondary hyperparathyroidism, anticoagulation, and prevention of complications in women with ESRD receiving long-term hemodialysis.
