Провести анализ результатов ABO-несовместимых трансплантаций почки от живого донора и сравнить с результатами трансплантаций почки от живого совместимого по группе крови донора и от трупного донора. Материалы и методы. В анализ включено 279 последовательно выполненных трансплантаций почки в период с 01.01.2011 по 01.06.2013 г. Пациенты были разделены на три группы в зависимости от типа донора: АBO-несовместимая трансплантация почки от родственного донора (нABO, n = 15), ABO-совместимая трансплантация почки от родственного донора (РТП, n = 114), трансплантация почки от трупного донора (ТТП, n = 150). В группе нABO реципиентам проводили предоперационную подготовку по схеме: Ритуксимаб + Иммуноадсорбци/Плазмаферез + Сывороточный человеческий иммуноглобулин в высокой дозе. В посттрансплантационном периоде все реципиенты получали трехкомпонентную иммуносупрессивную терапию (ингибитор кальциневрина + препараты микофеноловой кислоты + кортикостероиды). Результаты. Медиана длительности наблюдения за пациентами после трансплантации в группах нABO, РТП и ТТП составила 13, 15 и 12 месяцев соответственно. В группах РТП и нABO смертей реципиентов не было, в группе ТТП на разных сроках после трансплантации умерли 5 пациентов (3,3%). Частота острого отторжения трансплантата в группе нABO составила 13,3%, что достоверно не отличалось от групп РТП – 6,1% (p = 0,3040) и ТТП – 9,3% (p = 0,6177). Одно- и двухлетняя выживаемость трансплантатов составила 91,7 и 91,7% – в группе нABO; 97,3 и 93,1% – в группе РТП; 87,5 и 87,5% – в группе ТТП. Выживаемость трансплантатов в группе нABO достоверно не отличалась от выживаемости в группах РТП (log-rank test, p = 0,6938) и ТТП (log-rank test, p = 0,4868). Заключение. Выполнение несовместимых по группе крови трансплантаций почки является перспективным направлением трансплантологии и позволяет расширить пул родственных доноров, а также увеличить доступность трансплантации для ряда пациентов. Результаты несовместимых по группе крови трансплантаций при комплексном, индивидуальном подходе не уступают результатам совместимых по группе крови пересадок почки от родственного донора.</p></abstract><trans-abstract xml:lang="en"><p>Aim. Analysis of the results of living-related ABO-incompatible kidney transplantations (iABO) and comparison with results of ABO-compatible kidney transplantations from living-related (LD) and deceased donors (DD). Materials and Methods. 279 patients underwent kidney transplantations at our center between January 2011 and June 2013. The patients were divided into three groups: iABO (n = 15), LD (n = 114) and DD (n = 150). Patients from the iABO group underwent pre-transplant desensitization: Rituximab + Plasmapheresis / Immunoadsorption + High-dose IvIg. Maintenance immunosuppression was the same in all three groups: tacrolimus/cyclosporine A + MMF/ mycophenolic acid + steroids. Results. Medians of follow-up perion in the iABO, LD and DD groups were 13, 15 and 12 months, respectively. No death cases occurred in the iABO and LD groups. In the DD group five patients (3,3%) died on different times after Tx. Biopsy-proven acute rejection rate in iABO group was 13,3% and there were no significant differences in comparison with LD group (6,1%, p = 0,3040) and DD group (9,3%, p = 0,6177). One- and two-year graft survival in groups was 91,7% and 91,7% – iABO; 97,3% and 93,1% – LD; 87,5% and 87,5% – DD. No significant differences in graft survival between iABO vs LD (log-rank test p = 0,6938) and between iABO vs DD (log-rank test p = 0,4868) were found. Conclusion. The overcoming the ABO incompatibility barrier in living donor kidney transplantation is perspective trend in transplantology. It is perspective option for increase, at first, the pool of the living donors, and the access for kidney transplantation.
