Терапевтический плазмообмен (ТПО) рассматривается как метод, который, удаляя патогенные цитокины, может благоприятно влиять на течение тяжелых форм этого заболевания. Однако убедительные исследования этого вопроса до настоящего времени отсутствуют. Публикуются лишь описания отдельных клинических случаев или результаты небольших когортных исследований. Данные о применении ТПО у больных с почечной недостаточностью в литературе отсутствуют. Цель работы: оценить эффект ТПО при тяжелом течении СOVID-19 у больных с далеко зашедшей почечной недостаточностью. Материал и методы: в ретроспективное неконтролируемое наблюдательное одноцентровое исследование включено 211 больных (129 мужчин и 82 женщины) в возрасте в среднем 60,4±13,2 г. Из них 90,5% получали заместительную почечную терапию: 66,8% - гемодиализом (ГД), 9,5% - постоянным амбулаторным перитонеальным диализом (ПАПД), 14,2% были реципиентами почечного трансплантата в стадии умеренной или выраженной дисфункции, у 9,5% было острое повреждение почек на фоне хронической болезни почек, не требовавшее лечения диализом. Результаты: все больные разделены на 2 группы: 124 (58,8%) больных в возрасте 61,1±13,3 г, наблюдавшихся с 01.07.2020 по 16.12.2020, лечившихся ТПО (группа ТПО), и 87 (41,2%) пациентов в возрасте 63,5±14,0 лет, наблюдавшихся с 01.04.2020 по 30.06.2020, не получавших ТПО (контрольная группа). Состояние больных обеих групп при поступлении было тяжелым и примерно сопоставимым. В клинической картине болезни доминировала тяжелая пневмонии. Тяжесть поражения легких в группах статистически значимо не различалась. Значимые различия в показателях маркеров воспаления также отсутствовали: в обеих группах определялись высокий и не различающийся значимо уровень СРБ, ферритина, лактатдегидрогеназы, а также Д-Димера. Обе группы практически не различались по уровню лимфопении, тромбоцитопении и степени азотемии. Летальность в группе пациентов, не получавших ТПО, составила 73,5%, в контрольной группе - 45,16% (p&lt;0,001). Среди больных на хроническом диализе летальность в контрольной подгруппе была равна 74,6%, на фоне ТПО - 44,15% (р&lt;0,001). Заключение: терапевтический плазмообмен является эффективным подходом к лечению тяжелых вариантов COVID-19 у больных с далеко зашедшей почечной недостаточностью, которая, однако, может ограничивать его эффект в связи с повышением риска смерти вследствие уремии.</p></abstract><trans-abstract xml:lang="en"><p>Cytokine release syndrome plays a key role in the pathogenesis of COVID-19. Therapeutic plasma exchange (TPE) by removing pathogenic cytokines, can favorably influence the course of severe forms of this disease. However, conclusive studies on this issue are still lacking. Only descriptions of individual clinical cases or small cohort studies have been published. There are no data on the use of TPE in patients with renal failure in the literature. The study aims to evaluate the effect of TPE in the severe forms of COVID-19 in patients with advanced renal failure. Material and Methods: a retrospective, uncontrolled, observational study enrolled 211 patients aged 60,4±13,2. 90.5% of them received renal replacement therapy: 66.8% - hemodialysis, 9.5% - peritoneal dialysis, 14.2% renal transplant recipients with moderate to severe dysfunction, and 9.5% had acute kidney injury on chronic kidney disease that did not require dialysis treatment. Results. All patients were divided into 2 groups: 124 (58.8%) patients (treated from 01.07. to 15.12.2020), who received TPE (TPE group), and 87 (41.2%) patients (observed from 01.04. to 30.06.2020), who did not treat with TPE (control group). The condition of patients in both groups at admission was approximately comparable. The clinical picture of the disease was dominated by severe pneumonia. There were no significant differences in inflammatory markers: both groups had no significant differences in levels of CRP, ferritin, lactate dehydrogenase, or D-dimer. The groups also did not differ significantly in lymphopenia, thrombocytopenia, and azotemia. The mortality rate in the group of patients who did not receive TPE was 73.5%, while in the TPE group it was 45.16% (p&lt;0.001). Among patients on chronic dialysis, the mortality rate in the control subgroup was 74.6%, and in the TPE subgroup - 44.15% (p&lt;0.001). Conclusion: therapeutic plasma exchange is an efficient approach to the treatment of severe forms of COVID-19 in patients with advanced renal failure.
