В номере 3 нашего журнала опубликована статья В.М. Ермоленко, Н.Н. Филатовой, Д.О. Миленина и Е.А. Гореловой «Лечение вторичного гиперпаратиреоза при хронической болезни почек: новые концепции и инновационные препараты», представляющая собой обзор литературы, доступной на апрель 2012 г. К тому времени, когда этот материал был сдан в печать, появились новые и очень важные данные по лечению больных с ХБП 3–5 стадии, без которых представления о лечении вторичного гиперпаратиреоза в настоящее время были бы неполными. Профессор В.М. Ермоленко подготовил дополнение к предыдущей публикации, которое редакция журнала с удовольствием представляет Вашему вниманию. Фосфатонины, к которым принадлежит фактор роста фибробластов 23 (FGF23), являются важнейшими регуляторами баланса фосфора в организме. Наряду с влиянием на транспорт фосфора FGF23 ингибирует секрецию ПТГ и образование кальцитриола, запуская, по современным представлениям, развитие МКН у больных ХБП. Поэтому попытка назначить фосфатбиндеры пациентам с нарушенной функцией почек, но еще нормальным уровнем фосфата в крови выглядит вполне логичной. Однако назначение фосфатбиндеров больным с нормо- или умеренной гиперфосфатемией даже в краткосрочном исследовании неожиданно усугубили у больных кальцификацию коронарных сосудов и аорты. Таким образом, до проведения проверочных исследований не рекомендовано применение фосфатбиндеров у больных с преддиализной ХПН.</p></abstract><trans-abstract xml:lang="en"><p>The fibroblast growth factor 23 (FGF23) is an important regulator of phosphorus balance in the body. FGF23 influences transport of phosphorus and inhibits calcitriol by initiating the development of MBD in patients with CKD. So it seems logical to presribe phosphate binders to patients with renal dysfunction, who have normal blood phosphate level. However, attempts to use different phosphate binders in patients with moderate hyperphosphatemia some unexpected increase in vascular calcification have been observed.
