Захаровой Перевод выполнен по поручению комитета ВДП Проведение Всемирного дня почки 8 марта 2012 г. дает возможность оценить эффективность трансплантации почки как метода лечения терминальной стадии ХПН, превосходящего диализ, как по продолжительности, так и по качеству жизни пациентов, а также по экономической целесообразности. Будучи и лучше, и дешевле, это метод не стал доминирующим видом заместительной почечной терапии из-за различных препятствий, не позволяющих полностью заменить диализное лечение на трансплантацию. Барьеры на пути более широкого использования трансплантации как универсального метода лечения тХПН, включают в первую очередь экономические ограничения, из-за которых в некоторых странах пересадке почки уделяется меньшее внимание по сравнению с такими фундаментальными проблемами как обеспечение доступа к чистой воде, улучшение санитарных условий и вакцинация. Даже в странах с высоким уровнем экономического развития технические проблемы хирургического характера и последствия иммуносупрессии ограничивают число подходящих реципиентов, но главным фактором является недостаток донорских органов и нехватка подготовленных опытных хирургов, терапевтов и сестер. Решение этих проблем включает в себя целый ряд социальных, профессиональных, правительственных и политических аспектов. Всемирный день почки – это призыв обеспечить трансплантацию миллиону пациентов в год – всем тем, кто имеет право на такое лечение.</p></abstract><trans-abstract xml:lang="en"><p>World Kidney Day on March 8th 2012 provides a chance to reflect on the success of kidney transplantation as a therapy for end stage kidney disease that surpasses dialysis treatments both for the quality and quantity of life that it provides and for its cost effectiveness. Anything that is both cheaper and better, but is not actually the dominant therapy, must have other drawbacks that prevent replacement of all dialysis treatment by transplantation. The barriers to universal transplantation as the therapy for end stage kidney disease include the economic limitations which, in some countries place transplantation, appropriately, at a lower priority than public health fundamentals such as clean water, sanitation and vaccination. Even in high income countries the technical challenges of surgery and the consequences of immunosuppression restrict the number of suitable recipients, but the major finite restrictions on kidney transplantation rates are the shortage of donated organs and the limited medical, surgical and nursing workforces with the required expertise. These problems have solutions which involve the full range of societal, professional, governmental and political environments.
