Преждевременный атеросклероз при хронической почечной недостаточности

Аннотация: Введение Сердечно-сосудистые заболевания являются ведущей причиной смерти больных, страдающих прогрессирующей почечной недостаточностью, а также получающих заместительную почечную терапию. Показано, что риск атеросклероза у больных с хронической почечной недостаточностью (ХПН) и специально у тех из них, кто получает заместительную терапию, в 10-20 раз выше, чем в общей популяции. Поэтому пациенты с ХПН должны рассматриваться как группа высочайшего риска сердечно-сосудистых осложнений, которая нуждается в осуществлении соответствующих профилактических мер. Помощь больным с ХПН должна начинаться не в ее терминальной стадии или, тем более, после начала диализной терапии, но на том этапе, когда прогрессирование заболевания почек только диагностировано и выявлена начальная ХПН. Оптимальное лечение на этой стадии предупреждает вредные уремические воздействия на метаболизм, функции и структуры организма. Важность превентивных мер именно в ранней стадии ХПН, когда они наиболее эффективны как с медицинской, так и с экономической и социальной точек зрения, подчеркивается в Рекомендациях Национального Почечного Фонда США (National Kidney Foundation Task Force recommendations). Одной из главных задач ведения больных на этом этапе является ослабление воздействия как традиционных, так и нетрадиционных, специально связанных с ХПН факторов риска атеросклероза [1, 2, 3]. * Печатается с разрешения автора, перевод Н.А.
