Актуальные проблемы поздней дисфункции почечного аллотрансплантата
Аннотация
Трансплантация почки является в настоящее время методом выбора для большинства пациентов в терминальной стадии хронической почечной недостаточности. Прогресс в трансплантации почки достигался параллельно с усовершенствованием методов иммуносупрессии, что позволило значительно улучшить годичное выживание почечного аллотрансплантата (ПАТ) и добиться к концу девяностых годов прошлого столетия увеличения периода полужизни трупной почки до 19,5 лет [11]. Вместе с тем, долговременное выживание почечных аллотрансплантатов практически не изменилось за последние 10 лет [7, 10, 25]. Причиной этого, в первую очередь, является недостаточно исследованный патогенез поздней дисфункции ПАТ, в котором, по мнению большинства ученых, имеет место сложное взаимодействие иммунологических и неиммунологических составляющих [7, 33].
