Abstract
Up-to-date concept of mechanisms and pathogenesis of vascular calcification in chronic kidney disease are reviewed. Current view of the role of vitamin D in processes involved in development of vascular calcification is described. Different effects of vitamin D receptor activators (VDRA) on vascular calcification are presented. The other factors which affect the vascular calcification are described. pleiotropic effects of VDRAs are reviewed.

Abstract
Infectious complications after kidney transplantation occur in 36% of patients in the first year after transplantation and in in 52% of them in the first 6 months We review modern data concerning the infection appearance in the first month after kidney transplantation (mainly urinary tract infections) and those which are revealed later (viral and opportunistic infections). The predictors of the development of infections after kidney transplantation: hypoalbuminemia, low concentration of IgG1, decrease in the CD4 level, leucopenia, chronic infections, diabetes mellitus, the absence of antibodies to herpes-zoster-virus, CMV-positive status, HBV- and/or HCV are considered. The importance of the level of procalcitonin for differentiation of infectious complications and allograft rejection is emphasized. Standard measures for preventing infections after kidney transplantation: kotrimoksazol-forte for 6–12 months for all patients, valganciclovir for 3 first months for the CMV-positive donors independently on the CMV-status of the recipients and necessity of evaluation of the status of vaccination are discussed and evaluated.

Abstract
Patients who require long-term hemodialysis need long-term vascular access. An eight year retrospective review was undertaken to evaluate the patency rates of arteriovenous fistulae in patients with diabetes mellitus. The aim of this study was to compare the patency rates of wrist and elbow fistulae, and to propose the surgical tactics for establishment of adequate permanent vascular access for dialysis in diabetic patients.

Abstract
Decreased physical activity in CKD patients is widely prevalent and related with outcome worsening. Programs of intradialysis and interdialysis physical training are able to improve metabolic, hemodynamic and rehabilitation parameters. Efficacy of such programs depends on adherence to physical training schedule. The compliance index reflects patient’s education quality and surveillance efficiency and related with achieved results. During 12-months intradialysis training program of 10 stable HD patients improvement of some nutritional (triceps skin fold thickness, upper arm muscle circumference), inflammation (decrease of CRP logarithm), dialysis efficiency (phosphatemia) and quality of life (vitality and physical component summary of the KDQoL-SF questionnaire) parameters were found compared to control group of HD patient without physical training.

Abstract
Concentrations of serotonin and its metabolite, 5-hydroxyindoleacetic acid (5-HIAA), were determined using HPLC with ED in platelets and plasma of patients with CRF on program haemodialysis (HD) and in control group. Plasma adrenalin and noradrenalin levels were also determined. A significant decrease in platelet serotonin and an increase in serotonin and 5-HIAA levels in platelet free plasma were found in HD patients compared to control. Dialysis procedures decreased plasma levels of serotonin and 5-HIAA, but did not change platelet serotonin or plasma catecholamines. A correlation between serotonin and catecholamine values was found in HD patients, but not between plasma and platelet serotonin. Possible clinical manifestations of observed changes in blood monoamines are discussed. This work was supported with RFBR Grant 08-04-00951.
